Your initial assessment will give us the opportunity to meet with you to listen to your problem and begin finding solutions. The majority of our initial assessments last for one hour, allowing us the time to discuss your medical history and cause of injury so can leave the clinic with an informed understanding of the way forward. We will use a variety of physiotherapy assessment tools all leading to a comprehensive diagnosis. It is important for us at Formation Physiotherapy to ensure that there is time for you to also get treated as part of your initial assessment.

Yes. Our team of experienced Physiotherapists are available during working hours to chat through any questions you may have regarding your appointment.

We accept most major health insurance referrals at Formation Physiotherapy. When making your first appointment, please inform us of your insurance referral. It is the responsibility of the client to ensure they have correct authorisation from their private health insurance company prior to their physiotherapy assessment.

We understand that things come up and you may need to cancel or reschedule your appointment.
If you need to cancel an appointment we would kindly ask that you give us 24 hours notice. Any cancellation made after this will be charged the full appointment fee. Please contact the Clinic (028 9066 0836) and leave a message if no-one can answer your call; or send us an email (info@formationphysio.co.uk) and we will be happy to cancel or reschedule your appointment.
Missed appointments are considered to be cancelled appointments without adequate notice and therefore the full standard fee will be due.
If you arrive late for your appointment we will do our best to deliver your full appointment time but please bear in mind this may not always be possible if the diary is busy and the full appointment fee will stand.
